NLP Graphic Design Request Form

All Design items created will be subject to approval prior to printing/sending/uploading/etc. "Dates Needed By" are subject to a written approval of each design request and may take longer based on the receipt of sign-offs. Your approval is your authorization to proceed "as is". NLP, LLC or NLP Contractors will not be responsible for design, spelling, grammatical errors or incorrect information once project has been approved. You are also agreeing to pay vendors directly for any costs associated with printed pieces.
